Liability Release: By checking, I hereby acknowledge the awareness that my/my child’s participation of physical activities with Meaningful Life Foundation and those activities may involve the following: dancing, walking, exercising, sports, riding in a vehicle, volunteering in the community, participating in group activities at recreational and/or other vendor facilities, and being present and interacting with the community and partnered organizations. I have been informed and understand that my/my child’s participation in the aforementioned activities may expose me/my family to certain foreseeable and unforeseeable risks of damage and/or bodily injury, including serious bodily injury, where I may need to be hospitalized. I knowingly, freely, and voluntarily assume all risks and engage myself/my child in the participation of the above mentioned activities. I hereby release Meaningful Life Foundation, 973 N Young, and all partnering organizations and facilities in the community from any and all liability arising out my/my child’s participation of the above mentioned activities and waive my rights herein to assert any claim(s) for damages, bodily injury or serious bodily injury to the fullest extent allowed by law. I further agree that I will hold harmless Meaningful Life Foundation against and and all claim(s) for damages, bodily injury or serious bodily injury arising out of or in connection with my/my child’s participation in the above mentioned activities whether caused by negligence or otherwise. Upon execution of the form, I hereby unconditionally release, waive and discharge my right, whether by contract or under operation of law to file cause of action(s) or claim(s) which I may have against Meaningful Life Foundation now or in the future. I fully understand the terms set forth in this form, and I hereby execute the Release of Liability.
